Completed Application
Reference Check Authorizations (3)
Current Child Abuse Clearance
Current Criminal Clearance
FBI Clearance
Highest Degree (Original Viewed by WJS)
Official Transcript (Reflects Highest Degree)

In addition to the items above, a Complete Personnel File includes the following additional items:
Photo Identification Badge (Photo Taken at Interview)
Policies Signature Sheets (Signed at Training)
New Employee Training Verification Sheet (Completed at Training)
HIPAA Training Exam (Due One Week after Training)
Therapist Training Manual Exam (Due One Month after Training)
